Before scheduling your appointment it is required that you fill out our intake form. This is required ahead of time so that we can determine whether you can have the procedure safely and without any health risks.

 

This policy is in place for your benefit as we do not want you to schedule an appointment and come in only to find out that you have something in your medical history that could make colon hydrotherapy unsafe for you and prevent you from having the procedure.
